New Student Enrollment

Thank you for considering Edified Christian Preparatory Academy (ECA Prep) for your child's educational foundation. We invite you to visit us and take a personal tour of Edified Christian Academy if you haven’t already. Tours will be scheduled on a first-come, first-served basis. Please call our office for the scheduled tour dates and times.

We encourage you to familiarize yourself with all the contents of the application packet. Please be sure to: (1) Complete all requested forms. (2) Submit all requested information so that admission consideration will not be delayed. (3) Return all admissions and registration documents to the Admissions Office or submit them electronically below.

Registration forms are required to reserve your child’s space, and all fees are non-refundable. Please note that all documents listed below must be ready for upload to submit the enrollment application electronically.

  1. Download and sign the Enrollment Application.

  2. Download and complete the Health Inventory Form

  3. Download and complete the Blood Lead Testing Form (For Doctor)

  4. Download and complete the OCC Emergency Contact Form

  5. If applicable, download and have your child’s doctor complete the Medical Authorization Form if medication needs to be administered at school.

  6. Download and complete Release of Liability Waiver. 

  7. Copy of child’s birth certificate ready for upload

  8. Immunization Records ready for upload

  9. Child’s recent photo ready for upload

  10. Click this link for Student Recommendation Form to be completed by former Principal, Teacher, or Guidance Counselor.

  11. After steps 1-10 have been completed, please upload all documents to the portal via the upload box below. A verification email will be sent upon completion.  

  12. Click here to pay application fee. Please note this is application fee is separate from the enrollment fee listed on the enrollment packet.

  13. Click here to schedule your family interview. Remember both parents (unless legal obligation), siblings, and prospect student must be in attendance of family interview. Student will also be assessed at this time.
