Registration forms are required to reserve your child’s space, and all fees are non-refundable. Please note that all documents listed below must be ready for upload to submit the enrollment application electronically.
Download and sign the ReEnrollment Application.
Download and complete the Health Inventory Form
Download and complete the Blood Lead Testing Form (For Doctor)
Download and complete the OCC Emergency Contact Form
Download and complete the Release of Liability Wavier.
If applicable, download and have your child’s doctor complete the Medical Authorization Form if medication needs to be administered at school.
Copy of child’s birth certificate ready for upload
Immunization Records ready for upload
Child’s recent photo ready for upload
After steps 1-8 have been completed, please upload all documents to the portal via the upload box below. A verification email will be sent upon completion.
